Responsibilities:
Assist Registered Nurses in carrying out the care plan and performing specialized duties when necessary.
Communicate with all medical/clinical staff in regards of the patient's wellbeing (from assessment to medication, etc.
).
Traveling to patient's location (facility, residency etc.
) Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care.
Participates in the development of treatment plans and interventions.
Tansports and utilizes required medical equipment to and from designated locations (weighing approximately 25 - 30 lbs.
) Collaborates with teammates in setting of team goals, establishment of performance measures, and evaluation of team performance.
Documents assessment findings; treatment and interventions provided with patient/family responses in accordance with VNS Home Care policies, educate patient/family about treatment plan.
Participates in team meetings and other team activities to improve team goals, responsibilities, coordination of day to day service, reviews, support for new staff.
Qualifications 1 year of experience of LPN, preferably within Mental Health and/or Home Care setting.
Must have GED/High school diplomat NYS LPN BLS up to date Recommended Skills Basic Life Support Certified Nurse Practitioner Clinical Works Health Care Home Care Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
