Registered Dietitian

Provide collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life.
The professional educator supports and advocates for people affected by diabetes to optimize quality care by promoting self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.
Completes comprehensive assessments, interprets personal health data, develops an individualized care plan based on the patient' assessed needs and goals and promotes successful self-management.
Documents in the Electronic Health Record and outcomes data base according to the guidelines in a timely manner.
Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans.
Advocates for and supports technology-enabled medical nutrition therapy and care, in individual and population health services.
Collaborates with community partners to establish and maintain on-going support options.
Contributes to the achievement of established goals and objectives and adheres to policies, procedures, quality, and safety standards.
Complies with governmental and accreditation regulations.
Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
Participates in meetings, serves on committees, and represents the department in community outreach efforts as appropriate.
Performs other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge about nutrition guidelines in chronic disease management and disease self-management educational processes.
Knowledge about program management and the ability to analyze program data to drive quality outcomes.
Proficiency in various computer applications, including spreadsheets.
A high degree of organizational skills, ability to set priorities, manage multiple demands and the ability to complete tasks under strict timelines is required.
Effective written and verbal communication skills.
Maintains professional and personal integrity.
Bilingual in English and Chinese (Cantonese, Mandarin, or Fuzhou).
MINIMUM
Qualifications:
Bachelor's degree in dietetics/nutrition.
Valid license and registered by the Commission on Dietetic Registration.
1-year patient care experience preferred.
Additional training in chronic disease management or health-care related domains preferred.
Salary Range:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
